This is the award entry for The Jean Bishop Integrated Care Centre , entered by Hull Citycare in the Primary Care (New Build) category

The Jean Bishop Integrated Care Centre opened in May 2018 and is transforming the way frail elderly and patients with long-term conditions receive co-ordinated care in Hull to keep them healthy and living independently for as long as possible.

The project is the implementation of NHS Hull CCG's vision to improve the lives of the people of Hull by transforming the clinical model and integrating public services to provide a co-ordinated care plan for elderly and frail patients.

The Jean Bishop Integrated Care Centre is the new infrastructure that provides a dedicated community asset for comprehensive assessment, diagnostics, outpatient clinics and a wide range of physical therapy and rehabilitation facilities.

The centre also provides a new home for the East Hull Fire and Rescue crews, therefore the building design has integrated a fully-operational fire station for one appliance and the six-person crew. Several facilities such as the staff room kitchen and the gym are shared between health teams and the fire service.

The centre also provides hotdesk space for support staff, meetings room available to the community, and a cafe to cater for patients, staff and visitors to provide a fully-integrated service for the people of Hull.
